Thank you for helping us identify a deserving recipient for HERO’s Powerchair Giveaway.
Please review the information below before submitting your nomination.
Who Can Nominate?
- Family members or friends
- Caregivers or neighbors
- Healthcare professionals, social workers, or community leaders
- Individuals may also nominate themselves
Nominators must have the nominee’s permission to share their story.
Eligibility Criteria
- Nominee must reside in the HERO service area (Fargo–Moorhead region)
- Nominee must demonstrate a clear need for a powerchair (e.g., mobility challenges, limited insurance coverage or other barriers)
- Preference will be given to those who cannot easily obtain one through other funding or resources
- Nominee must be able to safely use the powerchair (with physician support if needed)
- Nominee must provide a copy of Doctor’s Rx for Powerchair if selected
Important Dates

Nominations Open: October 15, 2025 at 8:00 AM (CST) – Submit your nomination form online or by email.

Nominations Close: November 17, 2025 at 11:59 PM (CST) – All nominations must be received by this time.

Recipient Selection: November 18–19, 2025 – Committee reviews all submissions and selects the recipient.

Recipient Announced & Powerchair Presented: November 20, 2025 – HERO announces the recipient and presents the powerchair.
